Support built around the individual.

We're a family based group who needed better care and so we built it ourselves, we provide In Home and Community Supports so people with disabilities can live their Everyday Lives. We are currently accepting Individuals who are receiving Person and Family Directed Waiver (PFD) and Consolidated Waiver.

How does an individual receive our services ?

  1. The first inquiry is made by either the the person themselves (the Individual), the Family, or their Supports Coordinators to discuss the Individual's unique needs and our services.

  1. A formal referral is submitted by the Supports Coordinator to Every Step Wellness (ESW). The Individual Support Plan or ISP is shared with ESW.

  1. ESW will conduct a review of the ISP and the information obtained regarding the Individual's unique Needs and Preferences. After reviewing all information, ESW will let the Supports Coordinator know whether or not we can meet the Individual's Needs and their requests.

  1. When a Determination of Acceptance has been made, ESW begins to works very closely with the Individual, the Family and the Supports Coordinator during an intake period. Details of our service delivery and the Individual needs are discussed and agreed upon.

What we provide

In-home daily living support

Personal care, health and safety, meal preparation, household routines, and medication reminders— carried out in the person's own home, on their own schedule, by staff who know them.

Skill-building and independence

Practical goals and objectives chosen by the person and the support team — cooking, money management, communication, self-advocacy — worked on consistently.

Community integration

Supported access to local activities, social connections, and public spaces — not a day program, but the actual community the person lives in.

Transportation assistance

Getting to appointments, errands, and community events reliably — with a familiar support person, not a different driver every time.
